Disseminated tuberculosis in a child during the COVID-19 pandemic: a case report and literature review

Background Disseminated tuberculosis is an uncommon but devastating form of tuberculosis, possibly developing with the immune response patients. COVID-19 infection may produce immunosuppressive effect possible implications for dissemination. Case presentation A 17-year-old female patient a history tuberculous pleurisy presented to hospital high fever and life-threatening dyspnea after contracting infection. Her condition deteriorated rapidly grand mal epilepsy acute gastrointestinal bleeding grossly depressed CD4 T-cell count, which was indicative her profoundly immunosuppressed state. After identifying Mycobacterium in cerebrospinal fluid subcutaneous abscess left lower back, she diagnosed disseminated involving both lungs, central nervous system, terminal ileum, liver, bilateral adnexal tissue, soft tissue accordance chest abdominal CT. Empirical treatment initiated dexamethasone (5 mg/day) anti-tuberculosis regimen isoniazid, rifampicin, pyrazinamide, amikacin, meropenem, replaced faropenem hospital. The therapeutic considered satisfied second month follow-up. Conclusion To best our knowledge, we report first case Tuberculosis disseminate progress during pandemic, requiring more significant studies provide better diagnosis options co-infection.
